Please download, print and fill out the appropriate new patient kit prior to your first appointment. This reduces waiting time and helps keep our office running smoothly. The forms are in Acrobat PDF format.

If you are unable to keep your scheduled appointment with Dr. Kerri G. Hall, please call our office at 304-212-5423 least 24 business hours in advance so we can reschedule your appointment to a more convenient time. We look forward to seeing you at Suncrest Obstetrics & Gynecology.

Missed Appointment Fee:
Patients who miss two appointments will be required to pay a security deposit of $25.00 to reserve another appointment space. If the patient keeps the appointment the $25.00 deposit will be applied to the visit. Failure to keep appointment will result in forfeit of the $25.00 deposit.
